Frailty Learning Pathway

older person hands being held

Frailty is a recognised long-term condition. Whilst it can affect anyone at any age, it is primarily associated with older adults and its prevalence increases with age.

Frailty can be prevented and where identified some symptoms can be reversed or appropriately managed to reduce further deterioration and improve outcomes and quality of life, as well as reducing pressures on the health and social care system.

The Frailty programmes have been adapted by the Frailty Network NI in collaboration with HSC Clinical Education Centre, from the National Frailty Education Programme in Ireland (with kind permission from the creators). The programme takes a tiered learning approach  –

Tier 1 Frailty Awareness eLearning – developed by HSC Clinical Education Centre, is universally relevant to service users, family and carers and all health and care staff.
